Registration Information
The following is a list of
information that K-5 parents need to bring with them when enrolling
their child in an Alachua County school. Enrollment is not complete
until all documents have been supplied. It is the responsibility of
parents to provide documentation:

Two proofs of residence such
as utility bill (electric, cable, gas, or telephone) showing
parent's name and service address, property tax bill, mortgage
deed, sales contract for purchase of property, homestead
exemption receipt, or rental/lease agreements

Names and locations of schools
previously attended
In-County Transfers:
Out-of-County Transfers:

Certified copy of birth
certificate (FL Statute_232.03)

Immunization on valid Florida
Certificate HRS 680 form - Headstart, kindergarten, and first
grade must have two MMR's and Hepatitis B series

Student's Social Security card
Out-of-State Transfers and
First Time Entrants to Florida Schools:
-
All of the above, plus
kindergarten students must have Diphtheria-Tetanus-Pertussis
Series, Polio Series, and Varicella
-
Physical exam dated within
one-year of entry
